Valleybrook Member Contract

This is a copy of our membership contract.
Valleybrook Golf & Country Club operates a golf club and facility offering membership, and [Member Name] is desiring membership. Both parties enter into this agreement setting forth the terms and conditions of membership with each agreeing to be bound by certain terms.
If the application is approved, [Member Name] shall be and is hereby accepted as and is a member of Valleybrook Golf & Country Club with all privileges afforded as a result of membership. Membership privileges provide access to the golf course, all practice areas, clubhouse, and grill.
FINANCIAL RESPONSIBILITY
[Member Name] agrees to pay all recurring membership expenses, which are subject to change. If there is a price change, members will be notified at least 30 days in advance. The monthly membership dues do not cover any incidental expenses or charges incurred by the member as a result of the membership. Incidental expenses are charges incurred by the member and include, but are not limited to locker fees, food and beverage, cart fees and equipment, or clothing costs. All amounts owed by the member will be provided on a monthly statement and shall be paid in full by the due date provided. All disputes against the statement must be provided within 10 business days of the date on the statement.
In the event of lack of payment, [Member Name]'s membership with Valleybrook Golf & Country Club shall be subject to cancellation by the above facilities provided, however, that cancellation based on nonpayment shall not constitute a waiver of [Member Name]'s obligation to pay all outstanding amounts owed to Valleybrook Golf & Country Club.
CONDUCT
It is of the up most importance that the members and guests conduct themselves in an appropriate manner and obey all rules including the rules and regulations, etiquette, dress code, cart rules, and any other rules that may be implemented by the staff due to weather, or course conditions, under which the member and guest must obey. Serious violations of any rules or implemented rules concerning the conditions of the course may result in the termination of a member's contract solely at the discretion of management.
CONTRACT RENEWAL & CANCELLATION
This contract automatically renews every 12 months. Thirty (30) days’ notice is required to cancel membership, as well as a fee of three months’ membership dues to be paid within ninety (90) days. If notice is not received, [Member Name] is liable for all charges incurred.
MEDICAL LEAVE OF ABSENCE
Please contact our head golf professional regarding any requests for medical leave of absence or membership cancellation. Subject to approval, our policy allows members to pay an administrative fee of $125 (plus tax) instead of his/her standard monthly rate; this accommodation is offered for up to 90 days. Standard dues will be prorated at the end of the month if/when the member is ready to utilize their full benefits package, such as resuming play on the course. During medical leave, members will still be allowed to utilize practice facilities, be invited to member events, and enjoy exclusive social benefits, as long as the administrative fee is paid to remain active and in good standing. If a member will be out for more than 90 days, the head golf professional will discuss available options.
LEGAL CONDITIONS
The validity, interpretation, and performance of this contract shall be controlled by and construed under the law of the State of Tennessee.
In the event of a dispute between the parties as to the terms, obligations, and/or breach by either party of this contract, then the parties agree that the choice of venue for resolving the same shall be in the state court of Hamilton County.
The undersigned, having been given full and complete opportunity to review the entire agreement by evidence of electronic or written agreement in the member application constitutes agreement and a contract with Valleybrook Golf & Country Club.
